How does Milrinone work as an inodilator?

Milrinone inhibits PDE3, preventing cAMP breakdown. In cardiomyocytes, elevated cAMP increases calcium influx (positive inotropy). In vascular smooth muscle, elevated cAMP causes relaxation (vasodilation). This dual inotropic + vasodilatory effect is termed “inodilation.” Unlike beta-agonists, milrinone does not require functional beta-receptors, making it effective in receptor-downregulated heart failure.

Why is long-term oral Milrinone not used?

The PROMISE trial showed that oral milrinone increased mortality by 28% compared to placebo in chronic heart failure. Long-term PDE3 inhibition causes proarrhythmic effects (increased sudden death). Milrinone is therefore restricted to short-term IV use in acute settings only. This limitation applies to all PDE3 inhibitors.

What dose adjustment is needed in renal failure?

Milrinone is ~80% renally eliminated. Infusion rate must be reduced in renal impairment: CrCl 50 mL/min → reduce by 25%; CrCl 30 mL/min → reduce by 50%; CrCl 10 mL/min → reduce by 70%. Without adjustment, accumulation causes hypotension and arrhythmia.
