Why cannot Daptomycin be used for pneumonia?
Pulmonary surfactant (a complex of phospholipids and proteins lining alveoli) binds and inactivates daptomycin by sequestering its lipophilic decanoyl tail. This prevents membrane insertion into bacteria. Clinical trials showed inferior outcomes in community-acquired pneumonia. Daptomycin must not be used for any pulmonary infection.
